No political, economic, or cultural segment of society has escaped the universal impact of recent cataclysmic change. Physicians were no exception. During the 20th century, all members of society, including physicians, have experienced change of enormous speed and magnitude. Futurist Alvin Toffler noted that global societal transformation has created a of personal malaise that poses difficulty for both individual and group adaptation. Toffler further described current change as a fundamental shift and conflict in all aspects of civilization (how we live, (...) work, and relate to each other). Toffler also noted major alterations in the basis for distribution of societal power (who controls what, why, and how). One manifestation of recent change directly affecting all members of society, includingphysicians, has been the social transformation of medicine that has been a power shift from the individual physicians to control by organizations and management. Using robots that operate in the real world as opposed to computer simulations of animal behavior is a form of modeling that may provide some biological insights. However, since engineering principles and materials differ significantly from those used in biology, one should be extremely cautious in interpreting robot biomimicry as providing an explanation of biological mechanisms.

The demonstration that slow transport of clocks can be used to define simultaneity in inertial frames of reference leads to the question of whether clock transport can similarly be used in noninertial frames. It is shown that there are certain types of reference frames in which the clock-transport method cannot be used in a self-consistent manner. It is also shown that there are other types of noninertial frames in which the clock-transport method will succeed. The inaugural volume of the Pitt-Konstanz series, devoted to the work of philosopher Adolf Grünbaum, encompasses the philosophical problems of space, time, and cosmology, the nature of scientific methodology, and the foundations of psychoanalysis.

Dirac's classical electrodynamics countenances "preaccelerations" of charged particles at a time t as mathematical functions of external forces applied after the time t. These preaccelerations have been interpreted as evidence for physical retrocausation upon assuming that, in electrodynamics no less than in Newton's second law, external forces sustain an asymmetric causal relation to accelerations. And this retrocausal interpretation has just been defended against the critiques in (Grunbaum 1976), (Grunbaum and Janis, 1977 and 1978) by appeal to the formal assimilation of (...) the electrodynamic laws of motion to Newton's second law. It is argued below that this latest defense of the retrocausal interpretation is even more ill-founded than the prior ones in the literature. (shrink)

This essay explores the idea of experience and its epistemological and practical role in maintaining the health of a household among early modern English Royalists. A number of prominent royalists during the mid-seventeenth century British Civil Wars expended quite some effort in the collection of medical recipes, including Queen Henrietta Maria herself, as well as William and Margaret Cavendish, and the Talbot sisters—Elizabeth Grey and Alethea Howard. This essay looks at these Royalists and four of their collections: three published (Henrietta (...) Maria, Grey, Howard), and one manuscript (the Caven­dishes), in order to determine how they conceptualized experience and its role in medical practice. The claim that such recipe collections represent a new, anti-Aristotelian idea of experience as a specific, particular event is disputed through a quantitative and qualitative analysis of these collections. Instead, it is argued that there a number of related conceptions of experience found in these Royalist recipe collections, but the basic idea is one where experience indicates long experience or expertise, an idea that can traced back at least to humanist medicine of the Renaissance, and likely back to Galen. The third volume in the distinguished Progress in Self Psychology series brings together the most exciting issues in a rapidly expanding field. _Frontiers in Self Psychology_ is highlighted by sections dealing with self psychology and infancy and self psychology and the psychoses. Clinical contributions include several case studies along with a reconsideration of dream interpretation. Theoretical contributions span issues of gender identity, boundary formation, and the biological foundation of self psychology.

Volume 16 of Progress in Self Psychology, _How Responsive Should We Be_, illuminates the continuing tension between Kohut's emphasis on the patient's subjective experience and the post-Kohutian intersubjectivists' concern with the therapist's own subjectivity by focusing on issues of therapeutic posture and degree of therapist activity. Teicholz provides an integrative context for examining this tension by discussing affect as the common denominator underlying the analyst's empathy, subjectivity, and authenticity. Responses to the tension encompass the stance of intersubjective contextualism, advocacy of (...) "active responsiveness," and emphasis on the thorough-going bidirectionality of the analytic endeavor. Balancing these perspectives are a reprise on Kohut's concept of prolonged empathic immersion and a recasting of the issue of closeness and distance in the analytic relationship in terms of analysis of "the tie to the negative selfobject." Additional clinical contributions examine severe bulimia and suicidal rage as attempts at self-state regulation and address the self-reparative functions that inhere in the act of dreaming. Like previous volumes in the series, volume 16 demonstrates the applicability of self psychology to nonanalytic treatment modalities and clinical populations. Here, self psychology is brought to bear on psychotherapy with placed children, on work with adults with nonverbal learning disabilities, and on brief therapy. Rector's examination of twinship and religious experience, Hagman's elucidation of the creative process, and Siegel and Topel's experiment with supervision via the internet exemplify the ever-expanding explanatory range of self-psychological insights. Volume 17 of Progress in Self Psychology, _The Narcissistic Patient Revisited_, begins with the next installment of Strozier's "From the Kohut Archives": first publication of a fragment by Kohut on social class and self-formation and of four letters from his final decade. Taken together, Hazel Ipp's richly textured "Case of Gayle" and the commentaries that it elicits amount to a searching reexamination of narcissistic pathology and the therapeutic process. This illuminating reprise on the clinical phenomenology Kohut associated with "narcissistic personality (...) disorder" accounts for the volume title. The ability of modern self psychology to integrate central concepts from other theories gains expression in Teicholz's proposal for a two-tiered theory of intersubjectivity, in Brownlow's examination of the fear of intimacy, and in Garfield's model for the treatment of psychosis. The social relevance of self psychology comes to the fore in an examination of the experience of adopted children and an inquiry into the roots of mystical experience, both of which concern the ubiquity of the human longing for an idealized parent imago. Among contributions that bring self-psychological ideas to bear on the arts, Frank Lachmann's provocative "Words and Music," which links the history of music to the history of psychoanalytic thought in the quest for universal substrata of psychological experience, deserves special mention. Annette Lachmann's consideration of empathic failure among the characters in Shakespeare's _Othello_ and Silverstein's reflections on Schubert's self-states and selfobject needs in relation to the specific poems set to music in his Lieder round out a collection as richly broad based as the field of self psychology itself. A collection of thoughtul presentations on transference and countertransference highlights _The Realities of Transference_, Volume 6 in the Progress in Self Psychology series. The selfobject transferences receive special attention. Elsewhere in this volme, selfobject phenomena are examined in relation to the process of working through, the origins of ambition, the psychology of addiction, the psychodynamic consequences of AIDS, and creativity. An exploration of the selfobjects of the second half of life offers new insight into later development.

_New Therapeutic Visions_ begins with Lachmann and Beebe's developmental perspectives on representational and selfobject transferences, followed by commentaries. The philosophical dimension of self psychology is addressed by Riker, who looks at Kohut's bipolar theory of the self, and Kriegman, who examines the subjectivism-objectivism dialectic in self psychology from the standpoint of evolutionary biology. Clinical studies focus on self- and mutual regulation in relation to therapeutic action, countertransference and the curative process, (...) and the consequences of the negative selfobject in early character formation. A separate section of child studies includes a case study exemplifying a self-psychological approach to child therapy and an examination of pathological adaptation to childhood parent loss. :High-fidelity simulation is a relatively new teaching modality, which is gaining widespread acceptance in medical education. To date, dozens of studies have proven the usefulness of HFS in improving student, resident, and attending physician performance, with similar results in the allied health fields. Although many studies have analyzed the utility of simulation, few have investigated why it works. A recent study illustrated that permissive failure, leading to simulated mortality, is one HFS method that can improve long-term performance. Critics maintain, however, (...) that the use of simulated death is troubling and excessive. Given the controversy regarding simulated death, we consider the data about the educational value and the emotional harms associated with them, expecting that evidence could be useful in resolving the question. The goal of this narrative review is to explore the argument against simulated mortality and provide educators with an imperative as to why it can be safely utilized. (shrink)

Marina Sbisà has long advocated that we think of the illocutionary force of a speech act in terms of the act’s (predictable) systematic effects on the normative relationship between a speaker and her audience. Building on this idea, I argue that the hypothesis of distinctive speech act norms can be used to explain how participants in a conversation coordinate the normative expectations they have of one another in conversation. Such an explanation earns its keep by explaining how speakers render themselves (...) illocutionarily intelligible to their audiences—thereby enabling us to articulate the sense of mutuality that characterizes speech exchanges. One important upshot of my reflections is that we should reject both Austin’s thesis that the successful performance of an illocutionary act depends on audience uptake and Sbisà’s claim that Austin’s thesis is required if we are to make sense of the illocutionary dimension of speech in terms of an act’s systematic effects on participants’ normative expectations of one another. This book is the most comprehensive treatment available of one of the most urgent - and yet in some respects most neglected - problems in bioethics: decision-making for incompetents. Part I develops a general theory for making treatment and care decisions for patients who are not competent to decide for themselves. It provides an in-depth analysis of competence, articulates and defends a coherent set of principles to specify suitable surrogate decisionmakers and to guide their choices, examines the value of advance (...) directives, and investigates the role that considerations of cost ought to play in decisions concerning incompetents. Part II applies this theoretical framework to the distinctive problems of three important classes of individuals, many of whom are incompetent: minors, the elderly and psychiatric patients. The authors' approach combines a probing analysis of fundamental issues in ethical theory with a sensitive awareness of the concrete realities of health care institutions and the highly personal and individual character of difficult practical problems. In this paper I argue, first, that the most influential (and perhaps only acceptable) account of the epistemology of self-knowledge, developed and defended at great length in Wright (1989b) and (1989c) (among other places), leaves unanswered a question about the psychology of self-knowledge; second, that without an answer to this question about the psychology of self-knowledge, the epistemic account cannot be considered acceptable; and third, that neither Wright's own answer, nor an interpretation-based answer (based on a proposal from Jacobsen (1997)), (...) will suffice as an acceptable answer to the psychological question. My general ambition is thus to establish that more work is needed if we are to have a full account of self-knowledge in both its epistemological and psychological aspects. I conclude by suggesting how my thesis bears on those who aim to provide an empirical account of the cognition involved in self-knowledge. (shrink)

On the occasion of a first conference on Cognitive Science, it seems appropriate to review the basis of common understanding between the various disciplines. In my estimate, the most fundamental contribution so far of artificial intelligence and computer science to the joint enterprise of cognitive science has been the notion of a physical symbol system, i.e., the concept of a broad class of systems capable of having and manipulating symbols, yet realizable in the physical universe. The notion of symbol so (...) defined is internal to this concept, so it becomes a hypothesis that this notion of symbols includes the symbols that we humans use every day of our lives. In this paper we attempt systematically, but plainly, to lay out the nature of physical symbol systems. Such a review is in ways familiar, but not thereby useless. Restatement of fundamentals is an important exercise. (shrink)

This essay represents a reflection on the nature of shufa, the Chinese “art of writing,” and its ontological grounding as a continuous, “durational transcription,” of an inscriptional event, producing a phenomenology of “viewing.” This distinguishes it from ordinary writing (xiezi) in which attention is focused on the lexical meaning of the written characters (i.e., an experience of “reading”). Viewing a calligraphic inscription actually unfolding in time (i.e., as a dynamical structure or “temporal object event”), however, raises an interesting theoretical question (...) concerning the two complementary aspects of temporal consciousness of calligraphy as both duration and unity. This will be addressed in terms of recent discussions of a dynamical approach to Edmund Husserl’s theory of time consciousness. (shrink)

This essay is an overview of recent research aimed at establishing a link between environmental aesthetics and environmental ethics. I review the work of several prominent environmental philosophers and environmental aestheticians, spelling out some of the difficulties confronting various attempts to find such a link. While I argue that a case can be made for a connection between environmental aesthetics and environmental ethics concerning human‐created and human‐influenced environments, I find that there are a number of problems facing attempts to establish (...) a similar connection for natural or pristine environments. I examine some attempts to support such a connection, including each of two different accounts of the aesthetic appreciation of nature in contemporary Western environmental aesthetics as well as the union of these two accounts, sometimes called ecoaesthetics. I briefly discuss two Western versions of ecoaesthetics and then turn to research by Chinese aestheticians, who defend a more robust version of ecoaesthetics. I suggest that this latter work may succeed in connecting environmental aesthetics and environmental ethics, although not in exactly the way in which such a link has been pursued by Western philosophers.
